By checking the box to submit an entry to the Pumping Room Makeover Contest, you agree to the following, in addition to the Official Rules:

1. Ownership and License. You retain ownership of your submitted statement and photograph. You grant Aeroflow, Inc. and its affiliates a non-exclusive, worldwide, royalty-free license to use, reproduce, edit, and publish your submission for purposes of administering the Contest and for Sponsor's marketing and promotion of the Contest, without further compensation.

2. Your Certifications. You certify that you own the submission or have permission to submit it; that you have permission from the Nominated Organization to visit the space, photograph it, and submit the nomination; and that the submission contains no identifiable individuals, employee health information, or confidential business information — unless accompanied by a signed Individual Photo & Likeness Release for each identifiable person shown.

3. Content Restrictions. Your submission must not be unlawful, defamatory, obscene, threatening, harassing, hateful, or otherwise objectionable, and must not infringe any third party's rights.

4. Privacy. Personal information you provide as an Entrant (name, contact information) is used only to administer the Contest and is handled under Sponsor's contest-specific privacy notice. Information submitted with an entry, including name, email address, phone number, and any submitted content (Entry Data) will be collected, used, and processed by Sponsor solely for the purpose of administering this Contest, contacting potential winners, and fulfilling prizes. You were not identified or contacted as a potential Entrant through any use of your health information or your status as an Aeroflow customer or patient.

5. No Healthcare-Program Consideration or Referral Relationship. You certify that:

• You are not entering this Contest, and were not asked or encouraged to enter, in exchange for, or because of, any item or service Aeroflow has provided or may provide to you or to the Nominated Organization under Medicare, Medicaid, or any other health care program;

• Your entry is independent of any such relationship; and

• To your knowledge, the Nominated Organization is not a healthcare provider, supplier, referral source, or other entity in a position to refer patients or business to Sponsor. If this is not accurate, your entry is ineligible and will be disqualified — there is no process by which a referral-source nomination can be approved.

6. Electronic Acceptance. Checking the entry checkbox constitutes your electronic signature and acceptance of these Entry Terms and the Official Rules (Document 1), including the dispute-resolution terms in Document 1, Section 11.

7. Dispute Resolution. Disputes arising from your entry or these Entry Terms are governed by the arbitration, opt-out, and venue provisions in The Official Rules, Sections 11–12, which you accept by checking the box in Section 6 above.
